In this research guide you will learn how to get a COMPLETE, non-commercial, Statewide list of Assisted Living & Senior Care Facilities Provided by the State of Nevada.

At this time, there are 380 licensed Assisted Living Facilities in the State of Nevada.

You will quickly gain access to STATE-Supplied Assisted Living Facility data for every licensed facility in Nevada Including:

  • Inspections Reports, Statements of Deficiencies, and Plans for Correction
  • DIRECT facility contact information
  • Services provided (i.e. Alzheimer’s Care; Chronic Illness)

How To Find Assisted Living Facilities Statewide in Nevada using Commercial-Free State Resources

Bypassing the typical, ad-laden commercial senior care site and going to State-sponsored resources can save you time, hassle, and can also protect your personal information.  Additionally, State websites and databases are updated much more frequently than they typical for-profit site out there.

For Nevada, you can simply visit the State’s Department of Health & Human Services site (DHHS) where they offer an excellent database of the State’s Health Care Facilities, including:

  • Assisted Living Facilities
  • Residential Care Facilities
  • Skilled Nursing
  • Nursing Homes
  • Rural Clinics
  • Hospice Care

and much more.

To find Assisted Living, Click Here to visit the State’s search tool, and from there select “Health Facilities” from the Business Unit Menu, and select “Residential Facility for Groups” from the Credentials Menu.

NOTE:  You will see at the bottom of your output list the last time the database was updated.   

Initial Data Includes for each facility:

  • Facility Name
  • Credential Type
  • Credential Number
  • Facility Status
  • Expiration Date
  • Disciplinary Action
  • Address
  • Direct Phone Number
  • License First Issued Date
  • Primary (Administrator) Contact Name
  • Bed Count / Capacity

To view additional facility details, including inspections reports, click on the “View Detail” link for a specific facility.   From there, you will be taken to a page specific to that facility that will look like this:

Under the Endorsement section you will be able to see the types of services that the facility offers. 

How To View Inspections Reports for Assisted Living Facilities in Nevada

Quite simply, within the facility details page, there is a section dedicated to Inspections and Statements of Deficiencies.   There is also a section that will offer a “Grade” for some of the inspections.  

To view a particular inspections, and details therein, you can select SOD/POC or choose a document from the list under “Additional Statement of Deficiency and Plan of Correction.”  The document will likely be a .pdf that can be downloaded, and will look like this:

Assisted Living Rules & Regulations in Nevada

Anyone can access a copy of the State of Nevada’s Rules and Regulations for Assisted Living facilities.  We think that it is a great idea for families, seniors, residents and loved-ones to know where to find these rules and regulations, so they are familiar with what to expect, and to have a good idea of how facilities should be operated and managed. 

These rules and regulations also offer a great way for you to ask pointed, constructive questions to facilities on a variety of topics related to Health, Safety, Staffing, Food Service and more.

To view the Assisted Living Rules and Regulations for Nevada, visit the Nevada Division of Public and Behavioral Health site.  They have a page that looks like this:

You will be able to view a lot of information – and that is an understatement.   However, you will at least know where to go to find answers about almost anything pertaining to how a facility should conduct itself in a legal sense.
