The State of Iowa, and their Health Facilities Division, offers a handful of ways to file a complaint against a HealthCare Facility in Iowa, including Assisted Living Facilities.

Iowa’s Safest Facilities:

You may do so anonymously if you wish to protect your privacy and identity.

You’ll need to visit the Health Facilities website, where, in the left-hand column, you’ll see a “Complaints” link, along with a number of other helpful tools, and pages.

Going to the complaints section, you will see a description page detailing the types of complaints that can be investigated, as well as privacy information.

You’ll notice that it clearly says “Iowa law does not allow us to disclose the name of the complainant to anyone.”

This guarantees your anonymity.

The first way you can submit a complaint is through the Complaint Intake Line at

877-686-0027

You may also Fax your complaint, but I don’t think that many people would use this method any more

You may also submit your complaint by mail.

Remember, even though you are asked for your name and personal data, this information is to be treated as confidential, keeping your identity anonymous.

State law requires the investigation of any complaint concerning a 135C licensed health care facility be initiated within 20 working days.

    What are the rules on cleanliness in a care facility?
    Accura health in Ogden is so dirty. I was told they are updating everything. However 6mo into my employment i have yet to see anything being done. Please advise

    I believe the residential care facility needs to be investigated and it is call the Cedat Valley Ranch. Located in vinton iowa.

    Accura of manning isn’t following care plans for residents on their transfers we have residents who are hoyers and we couldn’t find the hoyer lift it was hidden down a hall that no resident lives on and no hoyer lifting pads were found in the residents room
